1963년 봄, 히로시마에서 명석구미와 신와카이 사이의 대리전 양상을 보이며 격렬한 내전이 벌어졌다. 명석계 우치모토카이(히로시마)와 히로노부구미(쿠레), 신와카이계 야마모리구미(히로시마) 간 대립이 첨예화되자 경찰이 폭력단 척결 '정상작전'을 펼쳤다.
이 와중에 히로노부구미와 야마모리 산하 우시마조직 간 대립도 심화되어 충돌과 보복 살인이 이어졌다. 우치모토카이 측도 오카지마 친구회 지원을 받아 야마모리구미를 공격했고, 결국 상호간 폭력적 대립으로 이어져 시민 공포가 극에 달했다.
경찰은 간부 일제 검거에 나섰고 야마모리 등이 체포되자, 오카지마구미는 명석구미와 함께 신와회 세력을 공략하며 내전 양상이 서일본 전역으로 확대됐다. 이 과정에서 17명의 사망자와 20여 명의 부상자가 발생하는 참혹한 사태가 벌어졌다.
