An opening shot of anarchy in the streets and somebody screams out loud and asks who is responsible for this chaos and then suddenly we get a shot of Mabuse who is introduced as Dr. Mabuse. Thus the answer to the screaming man's question. Director Fritz Lang claimed he filmed an opening sequence to this effect but film historians doubt any such sequence was ever actually filmed and thus was never in existence!
