舞台は総人口230万人弱、東京西部の大部分を占める巨大な都市。その都市は、人口の約8割が学生ということから、「学園都市」と呼ばれていた。 学園都市の生徒たちは、超能力を発現させるための特殊なカリキュラムに組み込まれ、発現した能力によって、「無能力者（レベル0）」から「超能力者（レベル5）」までの6段階で査定されていた。 主人公“上条当麻”（かみじょうとうま）は、学園都市の高校生だが、その評価は落第寸前のレベル0。なぜなら彼の右手には、異能の力ならなんでも打ち消す「幻想殺し（イマジンブレイカー）と呼ばれる能力が宿っていたからだ。そして彼はその能力のせいで常に不幸な人生を送っていた。 しかし、そんな日常に突如変化が訪れる。夏休みに入ったばかりの日、彼の部屋に、純白の修道服を着たシスターがいきなり空から降ってきたのだ！ 呆然とする“当麻”に向かって、“禁書目録（インデックス）”と名乗る少女――どう考えても偽名にしか思えない少女――は、「自分は10万3000冊の魔道書を記憶しており、そのせいで魔術師に狙われている」などとオカルト発言を連発。いぶかしむ“当麻”だったが、その日の夕方、彼の前にその“魔術師”が姿を現す。 突然の展開に戸惑う“当麻”に、その魔術師は「魔女狩りの王（イノケンティウス）」を作り出して攻撃を繰り出してくる！ やむなく“当麻”は、“インデックス”を守るべく異能を打ち消す力を持つ右手を握り、立ち向かうが……。
