劇情描述在末日之後的世界，一位父親與他的兩位兒子，躲在破舊的農舍裡為自己的性命而戰。
這是《福音戰士新劇場版》的第四部，也是最終版。美里以及她的反 NERV 组織 Wille 來到巴黎，這裡被核心染成红色，奇蹟號戰艦的船員降落在安全塔上，他們只有 720 秒能修復這個城市。NERV 的福音戰士群出現時，真理的 EVA 改八號機進行攔截。與此同時，真嗣、明日香和零（暫稱）在日本的土地上徘徊。
以第二部曲《福音戰士新劇場版：破》14年後為故事背景，明日香所駕駛的巨大類人型戰鬥機器人EVA3號機大暴走，並且被初號機擊毀後生死未卜。碇真嗣在戰鬥中使初號機完全覺醒，初號機頭上出現的天使光環，暗示人類將遭遇更可怕的第三次衝擊，第三新東京市亦發生前所未有的巨變。已40多歲的葛城美里（三石琴乃配音）為此帶領原屬特務機關NERV的部份人員，脫離「NERV」，並成立新組織「Wille」與之對抗，對所有隸屬NERV的EVA展開殲滅行動，葛城美里授命復活的明日香（宮村優子配音）和真裡（阪本真綾配音）奪回被封印在衛星軌道上的初號機和碇真嗣（緒方惠美配音）。 懵懂的碇真嗣跟隨NERV的所屬駕駛員綾波麗（林原惠美配音）登上突然入侵的EVA第九號機，真嗣在此處從擔任NERV最高司令官的父親-碇源堂（立木文彥配音）得知，自己將和美少年渚薰（石田彰配音）共同駕駛駕駛EVA第13號機。碇真嗣欲將NERV總部摧毀，而看似溫柔的渚薰在為他逐步揭開真相的同時，整個世界與人類社會亦發生翻天覆地的變化……
