Doctor Keitaro Shido, an acclaimed scientist, has been kidnapped by the terrorist organization Neo-Shocker in order to exploit his expertise in robotics technology. Desperate to escape, he convinces Neo-Shocker's command to allow him to operate on an injured camper. Explaining that he can turn the camper, Hiroshi Tsukuba into a powerful Neo-Shocker warrior, they allow Doctor Shido to operate. However, he quickly turns the tables on them, transforming Hiroshi not into an evil kaijin, but into the powerful warrior Kamen Rider, using his newfound abilities to combat the evil Neo-Shocker menace.

一萬年前，「神」創造了53隻稱之為「不死者」（UNDEAD）的生物，牠們互相戰鬥（劇中稱為BATTLE FIGHT），而最終的勝利者便可以統治地球，創造自己的理想世界，激戰之末人類UNDEAD打敗其餘52隻UNDEAD，創造了一個以人類為中心的和平世界。
一萬年後的現在，人類偶然發現記載UNDEAD戰鬥的遺跡（統稱人類基盤）和封印52隻UNDEAD的咭，為了研究這些遺跡，研究員成立了人類基盤史研究所（通稱BOARD）。BORAD的研究員廣瀨義人為了尋求令死去妻子復活的方法，竟意外解放了大部份UNDEAD。BOARD有鑒於此，立即開發出封印UNDEAD的系統，稱之為「騎士系統」：人類藉着和UNDEAD融合，變身成幪面超人對抗UNDEAD，劍崎一真（劍）及橘朔也（格連）分別被選為兩套幪面超人系統的着裝員。
劇情便由BOARD研究所被攻擊，橘朔也疑似擄走所長開始。而後延伸出不死者間的鬥爭與鬼牌不死者——相川始，尋找人類情感為主軸
本作中怪人是由800年的沉睡中覺醒的動物族，分別為鳥系、昆蟲系、貓科系、重力系，水棲系和恐龍系這六大族類。其中鳥類的幹部怪人（Ankh）因意外藉助使用瀕死狀態的泉刑警身體出現，與主角合作令其成為幪面超人OOO。
主角是名沒有夢想，沒有工作，也沒有家庭的男人（但在後期發現主角其實是某富貴人家的公子）。隨着戰鬥才漸漸對於活着這件事情有了確實的體會。而變身所使用的核心硬幣是作為本身能量來源的存在，沉睡的怪人因為醒來後發現作為自己身體能量源的代幣不足，因此開始追尋代幣。
怪人的終極目的，是希望搜集齊所有的代幣進行完全復活，成為萬物之「王」。幪面超人OOO為了粉碎這些怪人的野心而登場。
根據核心硬幣在Driver（變身器）擺放順序分別給與頭部、手臂、雙腳能力進行變身，通過打倒敵對怪人，可以獲得隱藏能力的硬幣。
初期變身的形態為老鷹、老虎、草蜢的結合體（鷹虎蝗），擁有獵鷹的視力和老虎的銳爪還有草蜢的跳躍力。
順帶一提，OOO的故事舞台是虛構地點「夢見町」。
Seven years ago, a meteorite struck the Shibuya district of Tokyo. Alien lifeforms known as the Worms, emerged from the meteorite and became a threat to humanity. Ever since, Souji Tendou has been training while waiting for the Kabuto Zecter so that he may properly take up the mantle of Kamen Rider Kabuto. Making many enemies while at the same time meeting other Riders with mysterious origins, Tendou attempts to accomplish his goal at all costs: to destroy all Worms that threaten humanity.
“Mirror World” exists symmetrically beyond an invisible mirror in space, and an exact copy of our human world, where only monsters can dwell. Evil monsters frequently cross the mirror and attack our world. Only Kamen Riders who make a contract with a specific monster and obtain special power from the monster can enter “Mirror World” and fight against evil monsters.
Kamen Rider: Dragon Knight is an American science fiction superhero television series that originally aired on The CW, as part of the CW4Kids programming block, from December 13, 2008 to December 26, 2009.
