Throughout the movie did everyone, especially the father knew that Alex wasn't actually there? Even at the end when Alex asks her Dad why he didn't send anything to Anna from Alex?

We'd have to believe that all that was Anna imagination and wasn't happening. Most of Alex scenes she's alone with Anna or talking only to her and not interacting with anybody else. So in the few scenes she interacts with anybody it's easy for filmmakers explain that Anna was just placing Alex there and imagining the other character was interacting when he wasn't. Yeah bad writing.
