La Dernière Terre s'est effondrée aux mains de Kenshiro, mais pour sauver le jeune Lord Doha, le puissant héros doit voyager à travers une terre hantée par des démons meurtriers dans cette suite de l'anime frappé Poing de l'étoile du Nord. Lord Doha est tombé malade, et s'il ne reçoit pas un sérum rare d'ici quelques jours, il mourra certainement. Bien que le sérum soit un voyage de deux jours dans les deux sens, Sara révèle qu'il y a un chemin plus court qui traverse Cliffland, mais il est hanté par des démons vicieux. Alors que Kenshiro cède courageusement la place à la redoutée Cliffland, un nouveau combattant nommé Seiji arrive en Terre Dernière, poursuivi par les méchants Clifflanders et jurant de récupérer l'héritage de Sanga.

The Last Land has crumbled at the hands of Kenshiro, but in order to save the young Lord Doha, the powerful hero must travel through a land haunted by murderous demons in this sequel to the hit anime Fist of the North Star. Lord Doha has fallen ill, and if he doesn't receive a rare serum within a few short days he will certainly die. Though the serum is a two-day trip in both directions, Sara reveals that there is a shorter path running through Cliffland but it is haunted by vicious demons. As Kenshiro bravely makes way for the dreaded Cliffland, a new fighter named Seiji arrives in Last Land pursued by evil Clifflanders and vowing to reclaim Sanga's legacy.

Nonostante la morte di Sanga, l'ombra della pazzia sembra non essersi allontanata da Lastland e dai suoi ignari abitanti: adesso devono infatti accogliere il giovane figlio del dittatore, Seiji, come nuovo tiranno. Quest'ultimo, profondamente adirato per essere stato in passato abbandonato dal genitore, ha deciso di impossessarsi dei suoi demoni, cosa che gli riesce facile data la momentanea assenza di Kenshiro, allontanatosi per cercare un siero immunizzante contro il tetano che sta uccidendo Bista. Durante la sua corsa contro il tempo, Kenshiro incontra Clifflander, misteriosi bonzi ritiratisi dal mondo per custodire uno spaventoso segreto. Proprio da costoro viene a conoscenza dell'esisitenza di un Clifflander fuggito e in possesso di una spaventosa tecnica di Kenpo, ovvero l'ambizioso e freddo Seiji. Sarà dunque l'Hokuto a ergersi a giudice del traditore?
